Our nurses put their passion into practice every day: supporting you to keep well and do the things you love. They’re often the first people you’ll talk to, and the ones who follow up with you to make sure things are going to plan.

What our nurses do
You'll see them for:
New patient appointments: a nurse will get to know you and take your history. They look after urgent care triage, vaccinations, urine testing, wound dressings, stitch removal, iron infusions, and follow-up calls after procedures or hospital stays.
But wait, there’s more...
Nurses are the lynch pins of care teams, helping you manage ongoing conditions. Heart health. Diabetes. Asthma. Bone health. They're trained to be proactive and catch problems early, to prevent illness becoming more serious and getting away on everyone. If you have a chronic condition, you'll meet with your nurse at least once a quarter. These aren't quick check-ins. You'll spend real time working through your health plan together - tracking your progress, adjusting what's not working, celebrating what is.
What happens in these appointments:
They'll track how you're doing (weight, blood pressure, the basics). They'll assess how your condition is progressing (checking your sight, mobility, foot health if needed). They'll ask about the things that matter. How you're eating, sleeping, moving. Whether you're smoking or drinking. How your medications are going. They'll screen for complications, coordinate with other services, and make sure you're not isolated or overwhelmed. If you're caring for someone else, they'll advocate for you and your family, too. A nurse’s attention is never just medical. They’re listeners and noticers, making sure all the pieces you need are coming together.

How it works
You, your doctor, and your nurse work as a team. When you see the same people consistently, they know you. They know what normal looks like for you. They are alert when something changes.
That’s when the trust you’ve built over time really matters. When things feel different, your team listens and acts quickly.
